In the EFSA document Compendium of botanicals reported to contain naturally occuring substances of possible concern for human health when used in food and food supplements, it was found that the cavalactones contained in methystine pepper have a hepatotoxic effect. According to the Committee on Herbal Medicinal Products (HMPC), there is sufficient evidence of toxicological concerns (carcinogenic potential in animals) for the herbal preparation. The HMPC noted the presence of reported spontaneous adverse drug reactions to the liver, including cases of liver failure qualifying for liver transplantation, as well as deaths. Such products have also been recalled in some Member States due to safety concerns related to liver toxicity (EMA, 2017).
